I'm having basically the same issue with MediaWiki. I guess we'll just have to wait until this gets fixed... Our error logs pretty much match up -- there is no entry for the 500 anywhere. All those are 404s. Let's hope this is fixed soon :) - Ben

If you're on PHP v.1, uploads are disabled. I believe they are enabled in version 2. I am also having a problem with a 500 error on my site with MediaWiki. I too am on Cossacks, and I am wondering if our problems are somehow related? Check your error log: do you see a reason for the error...

Hmm... I'm guessing combined is referring to Apache's combined log format, but I've never seen that before. Looks like a server configuration issue rather than something you've done.

Look in the error log and see what it says. That's always the first place to start :) If you post what it says I may be able to help further. But right now, it could be anything. - Ben
