That's probably because you aren't using your proper username and you haven't configured the password

Make sure you created the database, user, and then gave the user permissions in your cPanel.

Make sure you change your database name and login name.

Example:
Mysql database: CPANELUSERNAME_forum
Mysql Login: CPANELUSERNAME_PHPBB
Password: anything
