DNS name resquest

Hello,

I'm trying to transfer my domain name so the domain name will point towards my hosting account here. I spoke to the company who is dealing with my domain name and they said I would need the ISP TAG, name server details and/or the server IP address.

How would I get a hold of these? or could somebody please post the details here.
