installing phpBB3.0 need some info

i am trying to install phpBB3.0, and i need the Database information..

• The Database Type - the database you will be using.
• The Database server hostname or DSN - the address of the database server.
• The Database server port - the port of the database server (most of the time this is not needed).
• The Database name- the name of the database on the server.
• The Database username and Database password - the login data to access the database.

Type: MySQL
Server: localhost
Port: Unneeded

The database name and user will be in the format "ares623_name", where "name" is the database's/account's username. You can create a database though the MySQL wizard in your cPanel.
