linking PHP MySQL to Dreamweaver

hi guys

i'm trying to connect my database created in PHP MySQL to a form in my website that i've created in dreamweaver. When i try to define a connection it asks for the name of the MySQL server, my username and my password. Any ideas of where i can find these on my hosting control panel as ive tried all the ones i can think of.

They should be your username, password, and the web-site you are on.

Username: you database username(created by you in MySQL in Cpanel)
Password: your database username's password
