My add-on domain was removed

Yesterday, my add-on domain was removed. It was ptcrewards.us.to. When I tried re-adding it today it says it's already configured. Can you please reset it and add this add-on domain:
Domain Name: ptcrewards.us.to
Directory: public_html/ptcrewards
Username: something random
Password: something random
