Login to cPanel at
http://fallendarkness.exofire.net:2082/ with your username and password. If you search down the page, you'll find a link called MySQL Databases, click it. On the resulting page, there's a section at the very top to make a new database. Type a name (e.g. test) and click Create Database. Click the go back link to go back to the MySQL page.
Now scroll down a little further and there's a section to make a new MySQL user. Type a name (e.g. testuser) and a password, then click Create User. Click the go back link once the account has been made.
Even further down the MySQL page is a section where you can a MySQL user to a database. Select your user and database, and press Add. Check the 'All Privileges' box at the top of the table, and press Make Changes.
Now enter the information into your script's settings, and you're done
