Need Database Access Settings I can't Find them

Hello

I am using Php-Fusion with my website and I have set it up. But in order for it to launch I need the following information:

Database Hostname
Database Username
Database Password
Database Name
Table Prefix

Now I have setup a database using SQL and have added a user (me) to it. But when you go to here www.sfreakscs.x10hosting.com/setup.php I cannot get it to work.

This is to any administrators or support staff etc. Please, Please could you look up my account and find this information for me and enter it to that URL and click next. I hope this won't take to long and I would be extremely gratefull if a member of staff could do this for Me.

In your cPanel you should be able to create a database and that would be your info.

i no but im not sure whether it is

dex28_dex28 as username or
dex2828 :S

also i dont remember given the database a password ?
