Como dijo rackfenix, tienes que cambiar tu version de PHP a "Intermediate". Una vez echo esto, y aceptado la aplicación podrás usar la función mail() y otras mas. Saludos!

As host you need to put 'localhost'. The MySQL database, MySQL username and password are the ones you've set up on "cPanel > MySQL databases".

The user you see on PhpMyAdmin is not your MySQL user. As Corey has already say, you need to create go to cPanel > MySQL databases, and there you can: Create a MySQL database, Create a MySQL user & assign the user to the database.
