Copy the cv folder (just in case) to a backup. Can use FileManager 'Copy' feature. Use cPanel to drop subdomain cv. Use cPanel to create subdomain cv. Wait a bit, and then see if the DNS is updated.

Just use the 'Copy' command in FileManager to make a clone of the dir. Do you have an index file in the folder? If you have no index file and you have turned of indexing in .htaccess (even in folder above it), you will get Forbidden. Can you access any of the files in the subdomain directly?

Nope. It depends on the underlying OS, not on Apache. Apache on Windows is not case sensitive. And if you add mod_speling on a *nix machine, Apache becomes case insensitive.
