Cannot connect to MySQL database

I believe I have everything configured correctly (it was working) and now, all of a sudden, I cannot access data on my database. Further, PhpMyAdmin cannot connect to my database either. Prior to this, I made no changes in database users, passwords, or remote access status (or anything else for that matter).

Disregard on everycredit account--using friend's computer. Issue is on account called "kollmar".
