Cannot submit php upgrade request?

After filling in the reason and checking the advanced php mode upgrade I push submit but I get a blank page.

My reason for needing advanced php is because im trying to use top 10 clan list and clan roster feature on my website that includes its contents from www.elorating.com 's xml interface provided for clans to use on thier websites. Such as players names, ranks, ratings, games played, games won, games lost, favorite civilization, and country they live in.

You may apply for intermediate PHP Version. You can apply for Advanced PHP Version, only when you're told to do so by a staff here.
