Can't create MySQL databases or users

Hello,

I cannot seem to add a database or create new users. I can enter the information and submit it, and CPanel will confirm the creation of the database, but the database does not show up. Thank you.

Do you currently have any databases or database users, or are the lists in cPanel completely blank? Did you have any previously? If so, when did they disappear?
