CGI Perl email form 500 error

I uploaded a forms page, success page, and the FormMail cgi script from the following url:

http://www.scriptarchive.com/formmail.html

The form on this page:

http://jdwinfodesign.x10hosting.com/email_form.html

...is pointed at my version of the script (the only changes are the values for the recipient and the referrer):

http://jdwinfodesign.x10hosting.com/cgi-bin/FormMail-wT.pl

Instead of getting my success page, I get a 500 Internal Server error.

What is my next step?
