create mysql not working

Hi, I have gone thru the process of creating a database name and a db user on my account, however neither show up in the control panel, altho if i try to create them again, it says that they already exist

trying to create another differently named db or user result in the same non-entry to the control panel

therefore I am unable to use mySQL at this point as the control panel seems to be failing to show the details correctly in some way

your assistance is appreciated
