Form to email using PHP scripting

I have uploaded a form to submit information to my email. I was certain the script was correct but got a "mail disabled" message. After upgrading to intermediate levels. The message changed to "email not sent". It will not let me attatch the files so I will copy the scripts. My form looks very creative due to me wanting it to match my sites design. Can anyone see any scripting errors?

I got this script from the net which came with a txt file called "blockip.txt". I uploaded this just incase it needed it to work.

The message I get says "error sending e-mail!". Im lost not and have spent hours trying to sort it. Decided on getting help of those who know what they are talking about.

Sorry what do you mean by PHP function?
