Help, contact-form not found on server

My site located at:

have got some error on the contact form page. I think there are some conflicts on the server which I will like somebody to please help check .
And also I am trying to remove a php script called - contactform_free php from the server, I don't know how to do it.

I submitted the form at http://theonlypresenceisgod.com/contact-form/index.php with:
Your Name: test
Your Email: test@test.com
Subject: test
Message: test
and received the message "Thank you, your message has been sent."

Thanks, actually the problem was a new contact form i was trying to create. I have to delete all files on server. I have not re create the contact form. Another problem I am having now is this:

It shows a page I have already deleted but will not show the new page i created. The new page is shown in preview before publishing but will not show once was published.

I checked the server but could not locate where this page is pointing to on the server.

Your help will be appreciated.
