This was probably caused by allowing anonymous or bot users to upload files to your account. Make sure you disable uploads for users that have not been verified to be humans (eg, though a CAPTCHA service). You should also restrict file uploads to non-binary types whenever possible, and restrict binary types in size and file extension.
