I have on my raw log only (dir:access-logs) only text:
combined
combined
combined
combined
....
No ip or any other information

also latest visitor in cpanel shows only one visitor from IP: combined

Hmm... I'm guessing combined is referring to Apache's combined log format, but I've never seen that before. Looks like a server configuration issue rather than something you've done.
