Multiple Accounts Unblock Request

There is a link to unblock in the message that I received but this isn't taking unblock requests so I am trying here. If anyone has any information as to whether this is the right approach I would greatly appreciate it.

I do not knowingly have multiple accounts or use multiple accounts on x10host and would be grateful if it could be unblocked.
