Need Programing (Programming) Help

I've found one template for my website and there is one comment form. That template is simple HTML and CSS one. There is no code for that comment form. Can You help me by putting up the code here. I don't have any knowledge about PHP or JavaScript. but I think it may use mySQL database which require user name and password, just substitute username in place of username and password in place of password.

Without knowing what the form consists of, how it submits & what the database looks like there is no hope for us to help code the workings of a form. Maybe if you gave a link to the form we could work from there
