You probably don't have your meta tags established correctly or you just simply don't have your site adsense friendly.

Anyways, your site is okay, works good, tried it using my works computer. I like the simplicity, since the whole purpose of proxy sites are to insert a url and then your done right? You've done it with style and grace.

Add rich meta tags and keywords.....
Also add some content to your home page, just like "How to use the service blah blah blah".This should fix the problem with PSA.
I would suggest you to disable PSA from your adsense account profile.

Hmmm.. I have changed few meta keywords. I have also disabled PSA and given Alternate Ads URL there.. For time being, I have commented out the ad-code since the Alternate ADs needs some time to pick up.

Any comments on the usability of the site....?

I think the comments are not closed. This "--> " appear on the page.
And moreover the page looks empty. you can fill up some information about the site in the home page. The ads that popup when the navigation is done is quite nice.

About the PSAs, it's probably because you need to input the PIN or they've rejected your account, try logging in to Adsense and see if anything comes up about it.

No.. No.. My account is very much active.. No problem in it. Just now, I spoke to Google AdSense person. He told that AdSense crawler has not yet indexed my site. i dont know how much time it'll take.
