Upload script dead.

Hey guys, my upload script (for us to upload lecture notes etc onto our server for health studies) has died unexpectedly after 4 months of perfect operation.

The error is:

Not Acceptable

An appropriate representation of the requested resource /files/mednerds/upload.php could not be found on this server.
Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request.
Apache/2.2.9 (Unix) mod_ssl/2.2.9 OpenSSL/0.9.8b mod_mono/1.9 mod_auth_passthrough/2.1 mod_bwlimited/1.4 FrontPage/5.0.2.2635 Server at musoc.exofire.net Port 80 ________________


Have my permissions been cut back or something or have I been downgraded to lower php settings?
