I m trying to create a lead capture page, and I want to put a contact form app on my website to get name, phone #, address and email. I couldn't find any app in the c-panel that will allow me to do this simple thing. Does anyone know what I can do next?

As bmccoy11 said, you could use a fairly simple PHP script to accomplish this.
