Donation page leading to a blank screen?

Hi, on my website I setup a donation page, but whenever I click on the donation.php, it leads to a blank screen with just a standalone ":" in it. What seems to be the problem here and how do I fix it?

I would login to cpanel > click "Select PHP Version", click "Switch to PHP Settings" and make sure display_errors is ON and that error_reporting is set to E_ALL
** MAKE SURE YOU APPLY & SAVE THE SETTINGS **
Then reload your page to see any errors. You should generally get an error...
Don't forget to revert settings after

Nothin seems to work, I could use some more help. All there is, is a blank white screen and nothing comes up. It worked before, now it doesn't. Please help! Thanks.
