It doesn't matter where you put it, but it must be visible on every page. Just copy it and paste it into your HTML code. If you use some kind of software (example Drupal or SMF) you can find an add on for adding a banner ad.

The how to would depend on the forum software you use. You should be able to edit the templates somewhere in the admin control panel of the forum, all forum softs I've tried has had that option at least.

For the advanced package you can place it anywhere you like, footer template being a rather popular place to put ads could be an option.

For the corporate package, where it must be visible for a viewer without scrolling, the header template would be more suitable.
