Need Some Details for My Forum

I Upload My Forum to My Hosting and I Make File Config.php and Face Some Problem

1- Need Name (MASTER DATABASE SERVER NAME AND PORT)

2- Need (Forum Pacth)

This Path : /home/users/public_html/forums

Or

This Path: /home/public_html/Name Domain/forums

Hostname: localhost
If by Path you mean the document path , it would be (if you installed the files in forums )
/home/elnoby/public_html/forums
