Trying to Upgrade to Prem !

I am trying to upgrade to premuim with my account however I have been using the control panel with email and do not remember my username to put in the transfer box. I want the constant downtime to stop and willing pay for it!

btw this account for forum does not match the account name for the hosting my hosting is
