The suspension cannot -be- removed is the problem. The TOS quite clearly says proxies aren't allowed FOR ANY REASON. As such you were permanently suspended and cannot be unsuspended.

For a phishing site, you can't. Phishing is a major violation of the TOS and US Law, so you won't be getting your account back.
